e-goverenance services



District administration has started the jan suvidha kendra(common service center)
at sector 19 city magistrate office noida  to provide the  various services like domicle/cast/income certificates, application for ration card, birth certificate, death certificate etc. This is single window scheme where citizen can submit the application for his relevant service and a nominal fee of Rs 15 is deposited to get the computerized certificate

    Dear sir,
    This is Manoj kumar mishra i was very harassment from the department of JAN SUVIDHA KENDERA, SECTOR-19, NOIDA, DISTT. GAUTAM BUDH NAGAR (UTTAR PRADESH) 201301, when i was gone there first time he said to me that you are late so you fine Rs. 200 extra for making birth certificate. i give him at a time Rs. 200 cash from my pocket. and Rs. 15 extra give him for registration slip. He tell me that you come after ten days. when i was gone there after twelve days he tell me that server not running from internet. he tell me come after some days i am going to next many times but no any positive response i get from. they harassment me many times. no i tired from the department of JAN SUVIDHA KENDERA, SECTOR-19, NOIDA, DISTT. GAUTAM BUDH NAGAR (UTTAR PRADESH) 201301.

    PLEASE HELP ME WHAT CAN I DO?

      secondly I too applied for Birth Certificate of my son and it was totally hassle free, credit goes to senior official who took this initiative..And last but not the least the behavior of employees there was cordial.

      Rajeev.. as Rahul has said it is not the matter of one's english skills so please bade yourself from such lewd comments on one's ability. The message is clear that he faced some problem at this center.. you may differ on this but don't discourage anyone like this.. you may be good in English but we all are Indians and Hindi is our national language so don't feel any pride if you know English..If you can't help then don't give your useless tell-a-lie version.. I know this is an old post and I hope the current govt. will surely look into such issues of common people and will resolve them.

He tell me that you come after ten days. when i was gone there after twelve days he tell me that server not running from internet. he tell me come after some days i am going to next many times but no any positive response i get from. they harassment me many times. now i tired from the department of JAN SUVIDHA KENDERA, SECTOR-19, NOIDA, DISTT. GAUTAM BUDH NAGAR (UTTAR PRADESH) 201301. I was go there again on 3rd July for inquiry my child's birth certificate they asked the staff has change now you again deposit Rs. 25 and re-Submit the form. I do same as they said to me. They told me please take my no. and HE said please come after one month. his name is Mr. Arvind and his no. is 9999330525. when i was gone there after one month he asked that your certificate is gone to Dadri, and he asked that please contact to Mr. Bhasker in District Hospital- Sector-39, when i was gone there mr. Bhasker said to me please go in sector-19. Don't come here. You submit the form in sector-19 so please get your certificate in sector-19. I am a private service employee. so i cant not go every day to the Jan suvida kendra office. Now I am very tired to the Department of Jan Suvidha Kendra. Please make my Child's Birth Certificate.

    TO MAKE BIRTH CERTIFICATE IN NOIDA IS A VERY TOUGH WORK. IF YOU HAVE NOT MADE BIRTH CERTIFICATE OF YOUR CHILD WITHIN 21 DAYS THEN READY FOR HORSE RUNNING / HARROWING EXPERIENCES. WHEN I GO TO CMO OFFICE, NOIDA FOR BIRTH CERTIFICATES OF MY CHILDREN THEY SAID FIRST YOU TAKE APPROVAL/WRITTEN CONSENT OF CITY MAGISTRATE THEN WE THINK WHAT TO DO ? I DEPOSIT MY BIRTH CERTIFICATE APPLICATIONS AT CITY MAGISTRATE OFFICE, SECT-19, NOIDA WITH AFFIDAVIT, PVT HOSPITAL BIRTH CARD, PROOF ETC. AFTER SUBMISSION OF THE SAID DOCUMENT I HAVE FOUND TWO NUMBERS FROM THERE. THEREAFTER I WENT TO CMO OFFICE FOR BIRTH CERTIFICATE THEY SAID HERE YOUR NUMBER HAVE NOT COME, GO AND COME AFTER ONE WEEK. NOW MOVING FROM ONE PLACE TO ANOTHER IS GOING ON. MANY PEOPLE WANDERING SINCE ONE MONTH’S TIME FOR THIS PURPOSE. WHAT IS A PROCEDURE/SYSTEM ? GOVT BEATING THE DRUM OF E-SERVICES BUT IT IS VERY DIFFICULT FOR PUBLIC BECAUSE GOVT OFFICIALS ARE NOT TRYING TO MAKE IT EASY THEY MAKE IT MORE DIFFICULT.

    Hi Friends,

    Read the below if your child is more than one year old and you want to make his/her birth certificate.

    I am able to make birth certificate for my child (> 2 years). Just want to share the complete process. Since there are lot of questions and troubles one faces, in order to help you all and make your journey easy just follow the following steps.

    Step1 : Go to Sector 2,Noida SBI bank branch. For every year of your child you need to deposit Rs 10 as challan. In my case since my daughter is 2 years old, I deposited Rs 20 as challan. Important thing is that challan form is available at a tea shop just outside the gate of SBI branch. He charges Rs 10 for three copies of challan form.

    Step2: You would need a affadavit since child is more than a year old. Go to notary that sits outside noida authority office in Sector 6. For Rs 10 rupee stamp paper, I paid Rs 80. They already have the text material in computer and will print the same on stamp. Important thing is not to forget to put Rs 2 court fee stamp. You will get this from notary only. They will charge you Rs 5 for this.

    3) Go to Sector 19, Block B, Jan Suvidha kendra. Here you need to submit your documents which includes Affidavit(Step2), Challan Form(Step1), Birth form which you must have got from Hospital, Your photo id proof. You need to submit these inside the room, in front of main gate. Sometime the person who receives the form try to get some money, no need to pay anything. He doesn't have any powers.

    4) After 1-2 days, again go to Sector 19, Jan Suvidha Kendra, to get the dispatch no of your forms. Actually from Sector 19 your documents will be sent to Sector 39, Mahila thana, Block5. You need the dispatch no against which your forms have been sent to sector 39. Just note down this dispatch no.

    5) After 10 days go to Block no 5, Mahila thana, Sector 39. ( If you are coming from Sector 71 towards Noida Metro station, then just take left from Noida metro station. Take U turn from first lights and the mahila thana is at ur left side.) Park your vehicle on main road itself. Go inside, only helpful person is Mr Sunil, you will find him helping people. Just give your dispatch no which you got from Sector 19. He will handover u forms and ask you to get them sign from one room there, after signing he will note down this in his register and give u to all the documents.

    6) Now take all the documents and go back to Jan Suvidha kendra, Sector 19 Noida and submit the docs. They will give you slip and will ask you to come back after 20 days.

    7) Go to Sector 19, suvidha kendra after 20 days with the slip and get the birth certificates....

      Some of my experience in this process:
      - You need to write an application to the City Magistrate, Noida stating the reason why you could not get the certificate registered within 21 days (any reason would do). A Rs.2 court fee stamp is also required on this application - so when you buy the stamp paper and court fee stamp for that, buy additional stamp for the application

      - the text of the affidavit is simple and you can print yourself and then get it signed by the notary the text is as follows:

      I, [Parent's Name] S/o [Parent's Father's Name], R/o [Parent's Address], do hereby solemnly affirm and declare as under:

      1. That the above name and address is true and correct

      3. That his/her place of birth is [name and address of the hospital where your child was born]
      4. That his/her mother's name is [Mother's Name]

      VERIFICATION:
      Verified that the above contents of this affidavit are tru and correct to the best of my knowledge and belief

      - The Challan is Treasury Form 209(1) or Form No. 43a(1) and need not be purchased, it can be downloaded from below links and printed on a simple A4 paper and filled in three copies:

      English: http://ekosh.uk.gov.in/upload/forms/Form-4.pdf

      For point 8 in the form (Account Head or Lekhashirshak), the code will be: 0210 - 04 - 800 - 99
      * note that though it asks for 13 digit code you will fill only 11 digits - leave last 2 boxes for detail head blank. You can confirm this at the SBI sec 2 branch challan counter. bank will keep 2 copies and will return one stamped copy to you

      - once you have all these document you need to submit them in room no. 3 at the sector 19 of Jan Suvidha kendra

    My father Sh. Ishwar Dayal Gupta expired on the morning of 01/04/2013 in Sector,82, NOIDA. He was cremated at the Nigam Bodh Ghat, Yamuna bazar, Delhi. Since then I have been trying to get his death certificate from the MCD office at Delhi. But they now told me that the certificate will be available in NOIDA only.       Death certificate will be made based on place of death, in Delhi it is quite simple just fill form (available online) and submit with cremation receipt at citizen service beaurau. After 21 days you will get DC RS 10 / copy

    My daughter born in 17 dec 2007.I have the birth certificate which was issue in 2008 from Jan Subhidha Center ,Sec 19 Noida which is Hand written.I want to issue in computerize it.They told me you fulfill the all formalities like Callahan fee at bank Rs 10 for every year. Affidavit at Rs 10 Stamp paper verify it from Magistrate office in sec 19 after that go to sec 39 CMO office it is the process of 15 to 20 days. Any body help me. It means that they have no record before the 2008. because computerize certificate started after 2010.It is very bad. "Digital india"

    Hi All,

    My name is Sandeep Kumar Manna. I was born on 25th Nov 1994 in ESIC Noida Sec-24. The problem is i have birth certificate in which my mother name has mentioned as a nick name now in my every document and my mother's document name is different. So, request you to what are the possibilites to get this corrected on urgent basis.

    I have submitted my form 6 (WITH ALL ENCLOSURES AND PHOTOS)for inclusion in the Voter's List to the BLO in Chaukhani Village Primary School Teacher for Sector 70, PAN OASIS, NOIDA-two months back. But to day I have checked on line the Voter's List of Sector 70, Part.411 (page 26-29) but our faMily name has not been included. The names of our family are as under :-
    1. VIRENDER KUMAR JAIN, 68 YEARS.
    2. SMT NEELAM JAIN, 60 YEARS
    3. MR. KARAN JAIN-30 YEARS
    4. MRS. SHRUTI BANSAL JAIN-31 YEARS.
    5. MS. KANIKA JAIN - 28 YEARS.

    Kindly check why our names have not been included and get then included for issue of Voter's I.Cards.

    Plz tell me the procedure of name change in birth certificate of my 1 yr old baby. I visited to nagar nigam office located in sec-39 but they denied by saying that'what's the need of name change..its a gud name' and they didn't tell me the procedure'.

    my sister has transferd her propety in ludhiana to her brother.sir she is aresident of sec 20,noida. sir now she needs a affidavi regarding that she will not revoke the said property.
